ECG of the Week - 6th August 2018 - Interpretation

This ECG is from a 78 yr old female with a history if hypertension and hypercholesterolaemia who presented with 2 hours of central chest pain.

Rate:
  • 60 bpm
Rhythm:
  • Regular
  • Sinus rhythm
Axis:
  • Normal
Intervals:
  • PR - Normal (~160ms)
  • QRS - Normal (100ms)
  • QT - 410ms
Additional:
  • ST Elevation lead II, III, aVF, V5-6 (~1mm)
  • ST Depression leads V1-3
    • Flat morphology
  • Biphasic T wave V2
  • T wave inversion lead aVL
Interpretation:
  • Infero-postero-lateral STEMI

What happened ?

The patient was transferred for urgent angiogram which showed an occluded circumflex which was stented.
