Perhaps ventriculophasic sinus arrhythmia?Christopherhttps://www.blogger.com/profile/11415988855392944633noreply@blogger.comtag:blogger.com,1999:blog-1199060416202745408.post-2938442140564061332012-09-27T10:19:34.113+08:002012-09-27T10:19:34.113+08:00Christopher,

Thoughts?Dave Bhttps://www.blogger.com/profile/04348546121665459931noreply@blogger.comtag:blogger.com,1999:blog-1199060416202745408.post-79193014827205283302012-09-21T22:49:48.876+08:002012-09-21T22:49:48.876+08:00Rhythm: sinus tachycardia at 120 bpm with a comple...Rhythm: sinus tachycardia at 120 bpm with a complete heart block and a junctional escape at 50 bpm. A rhythm strip would help determine if there were capture at any point, but I doubt it as the R-R remains very regular.

Interpretation: Inferior wall myocardial infarction (ST-E II/III/aVF/V6, ST-D I/aVL) with possible posterior involvement (ST-D V2-V4). Likely proximal RCA occlusion (ST-E III > II).

The efficacy of this patient's junctional rhythm is going to determine whether pacing gets utilized. I doubt atropine will help as it appears there is little vagal mediation of the sinus rate.Christopherhttps://www.blogger.com/profile/11415988855392944633noreply@blogger.com
